Working in partnership with your physician, Accredited-We Are Home Care is a personal home health care company located in southeastern Michigan, serving 16 counties in lower Michigan. Accredited is owned and managed by a licensed physical therapist who has been actively working in home health care for more than 40 years. Our personalized skilled nursing care, physical therapy, speech therapy and occupational therapy is provided by our licensed, experienced staff. Accredited – We Are Home Care has a nurse on call 24 hours a day 365 days a year for that extra personal service our clients deserve. We accept all major insurance including Blue Cross and Blue Shield, Workers Compensation, Medicare and Medicare Advantage. Skilled Nursing

Skilled nursing services are provided by a registered nurse (RN) or licensed practical nurse (LPN) under the direction of a supervising nurse. Your nurse is your health care team leader and coordinates all the services you receive, communicating with your physician to update your plan of care.

Physical Therapy

Physical Therapy (PT) services are provided by licensed professional staff in accordance with your physician’s plan of care. Accredited only employs PT’s who are graduates of colleges accredited by the American Physical Therapy Association. Patients requiring physical therapy services are those presenting intractable pain and physical dependence on others for transfer, mobility, and ambulation needs.

Occupational Therapy

Occupational Therapy (OT) is provided by a qualified Occupational Therapist in accordance with your physician’s written orders.

Speech Language Pathology

Speech Language-Pathology (SLP) is provided by a qualified Speech Therapist in accordance with your physician’s written orders. The therapist will include family members, as appropriate, to properly supplement oral and communication exercises in the home.

Social Services

Accredited’s Medical Social Workers provide assistance to home-bound patients who have social and emotional issues related to their medical needs. Our social workers can also assist with nursing home placement in the event that becomes necessary.
