Home Health Aide

The Home Health Aide is your personal caregiver. Visits are made to provide hands on, and unique personal care. These services may include, but are not limited to:

  • Bathing
  • Dressing
  • Hair care
  • Finger Nails
  • Oral hygiene
  • Shaving (electric razor)
  • Foot care
  • Changing bed linens (for bed bound patients)
  • Feeding
  • Assistance with elimination
  • Assistance with walking
  • Assistance with transfers
  • Routine catheter care
  • Routine colostomy care
  • Changing position in bed
  • Simple dressing changes.

Skilled Nursing

Skilled nursing services are provided by a registered nurse (RN) or licensed practical nurse (LPN) under the direction of a supervising nurse. The nurse is your health care team leader and coordinates all the services you receive. The nurse communicates with your physician to update your plan of care. AHC nurses also provides comprehensive laboratory services, such as blood tests, urinalysis, and more.

Speech Language Pathology

Speech language-pathology (SLP) is provided in accordance with the physician’s written orders.
SLP services are provided by a qualified Speech Therapist. The therapist will include family members, as appropriate, to properly supplement oral and communication exercises in the home. Patient's requiring Speech Therapy include, but are not limited to:

  • Articulation disorders
  • Cerebral Palsy
  • Cerebral vascular accident
  • Cleft lip and palate Head injury
  • Laryngectomy-esophalgeal speech
  • Training in the use of mechanical

Occupational Therapy

Occupational therapy (OT) is provided in accordance with a physician’s written orders. OT services are provided by a qualified Occupational Therapist. Patients requiring Occupational Therapy include, but is not limited to:

  • Accident, i.e. fractured hip Amputation of an extremity or digits Arthritis Cerebral Vascular accident
  • Neuro-muscular diseases
  • Psychiatric illnesses

The Occupational Therapy Program May Include, but is not limited to:

  • Assistance with feeding programs Designing,
  • Fabricating and fitting of orthotic and self-help devices
  • Perception,
  • Strengthening and balance training
  • Restorative therapy for improving independence in activities for daily living
  • Teaching compensating techniques

Physical Therapy

Physical therapy is provided in accordance with the physician's plan of care. Physical therapy is provided by only licensed staff who are graduates of colleges accredited by the American Physical Therapy Association. Patients requiring physical therapy service are those presenting intractable pain and physically dependence on others for transfer, mobility, and ambulation needs. Patients discharged from hospitals, SNE, rehabilitation centers, out patient surgeries, or doctor identified mobility needs are all eligible.

The physical therapy program can include but is not limited to:
  • Gait Training and progression of wheelchair, walker, cane, and independence on multiple living surfaces.
  • Functional Activities for transfer training and safety.
  • Manual Techniques for joint mobilization and myofacial release with stretching and relaxation techniques.
  • Neuromuscular Re-Education for improving balance, equilibrium, and home safety from falls.
  • Therapeutic Exercises for stabilization, strengthening, and improving of endurance for full quality of life.
  • Home Exercise Program for your individual mobility, safety, and pain management needs.
  • Ultra Sound for tissue restrictions and pain management needs.
  • Coordination of durable medical equipment.
  • Coordination of your individual treatment with your physician and AHC professional staff.

Social Services

The Medical Social Worker provides assistance to home-bound patients who have social and emotional issues related to their medical needs. These problems may include, but are not limited to:

  • Home safety
  • Community resources
The social worker can also assist with nursing home placement in the event that becomes necessary.
Specific services and duties include, but are not limited to:
  • Accessing community and financial resources
  • Assessment of the patient’s living environment and financial status in relation to the disease process
  • Assisting the members of the health care team and the patient’s family to understand the significant social and emotional factors related to the patient’s health problems
  • Locating alternate living arrangements
