Accredited Home Care has access to all medical supplies, therapy items, and equipment. We will coordinate delivery of supplies to your home
The Home Health Aide is your personal caregiver.
Visits are made to provide hands on, personal care.
These services may include, but are not limited to:
Bathing
Dressing
Hair care
Finger Nails
Oral hygiene
Shaving
Foot care
Changing bed linens
Feeding
Assistance with elimination
Assistance with walking
Assistance with transfers
Routine catheter care
Routine colostomy care
Changing position in bed
Simple dressing changes.
Skilled nursing services are provided by a registered nurse (RN)
or
licensed practical nurse (LPN)
under the direction of a supervising nurse.
The nurse is your health care team leader and coordinates all the services you receive.
The nurse communicates with your physician to update your plan of care.
AHC also provides comprehensive laboratory services.
Patients requiring Skilled Nursing care include:
Cancer
Cardiovascular disease
Cerebral Vascular accidents
Diabetes
Digestive problems
Fractures
Wounds
Skilled Nursing can include but is not limited to:
Catheter insertion
Diabetes management
Evaluation and assessment
Medication administration
Patient and family teachings
Stoma care
Wound care
Speech language-pathology (SLP) is provided in accordance with the physician’s written orders.
SLP services are provided by a qualified Speech Therapist.
The therapist will include family members, as appropriate, to properly supplement oral and
communication exercises in the home.
Patient's requiring Speech Therapy include:
Articulation disorders
Cerebral Palsy
Cerebral vascular accident
Cleft lip and palate
Head injury
Laryngectomy-esophalgeal speech or training in the use of mechanical device
Neuro-muscular impairments
Parkinson’s disease
Stuttering
Swallowing dysfunction
Tongue thrust
Voice and language disorders
Speech Therapy can include, but is not limited to:
Communication programs
Diagnostic and evaluation services
Instruction for family, friends, or home health aide in aspects of speech language exercises
Oral-muscular exercises
Occupational therapy (OT) is provided in accordance with a physician’s written orders.
OT services are provided by a qualified Occupational Therapist.
Patients Requiring Occupational Therapy include:
Accident, i.e. fractured hip
Amputation of an extremity or digits
Arthritis
Cerebral Vascular accident
Neuro-muscular diseases
Psychiatric illnesses
The Occupational Therapy Program May Include but is not limited to:
Assistance with feeding programs
Designing, fabricating and fitting of orthotic and self-help devices
Perception, strengthening and balance training
Restorative therapy for improving independence in activities for daily living
Teaching compensating techniques
Physical therapy services are provided in accordance with the physician’s written orders.
PT services are provided by a qualified Physical Therapist.
Patient's requiring Physical Therapy include:
Accident, i.e. fractures
Amputation
Arthritis
Back pain
Cerebral Vascular accident
Neuro-muscular diseases
The Physical Therapy program can include but is not limited to:
Diagnostic and evaluation services
Gait training
Instruction and training in the use of prosthetic or orthotic devices
Muscle strengthening programs
Pain management
Restoration therapy and rehabilitation following fractures, amputation, shoulder, hip, knee surgery, or stroke
Supplying needed equipment, instruction and exercise programs for whirlpool, walker, cane, crutches, and home traction
Therapeutic exercises
The Medical Social Worker provides assistance to home-bound patients who have social and emotional issues related to their medical needs.
These problems may include financial, transportation, and family problems, and decision making.
The social worker can also assist with nursing home placement in the event that becomes necessary.
Specific services and duties include:
Accessing community and financial resources
Assessment of the patient’s living environment and financial status in relation to the disease process
Assisting the members of the health care team and the patient’s family to understand the significant social and emotional factors related to the patient’s health problems
Locating alternate living arrangements
Mediate family conflict and assist in the decision making process
Psychosocial assessment and counseling interventions that contribute significantly to the treatment of the patient’s medical condition
Short term counseling following changes in the patient’s life situation
Infusion therapy (IV's) is performed in the home by AHC's knowledgeable staff (PICC lines, midlines, ports and other central line catheters).
Medicare and Blue Cross Blue Shield of Michigan will pay to educate the patient and family members in this procedure.
